CHP consists of nine regions that provide healthcare services to people in Indiana/Kentucky; eastern Tennessee; southwest Ohio; western Ohio; west central Ohio; northwest Ohio/southern Michigan; north central Ohio; northeast Ohio; and northeast Pennsylvania.

Each region connects separate facilities and services together into an integrated continuum of care that serves area residents at every state of life. This continuum of care generally includes acute care; long-term care; housing for the elderly; home care; hospice; rehabilitation; wellness classes and exercise facilities; and outreach services to help those who are poor and disadvantaged.
