Catholic Health Partners (CHP) is implementing the Epic EHR in all of its hospitals and employed physician practices. The initiative is called CarePATH, and the “PATH” in CarePATH stands for Patient-centered Access to Team-based Healthcare.
CarePATH is a comprehensive, safety-oriented, patient-centered clinical information system. With CarePATH, each patient will have a single, continuously updated electronic health record.
Consequently, our clinical teams will have instant, comprehensive access to information about the patients in their care. CarePATH will connect them to the latest web-based knowledge in real-time. It will also allow them to access evidence-based protocols and best practices for their patients and to evaluate their own performance against those best practices.
Specific benefits for your practice include enhanced patient safety since CarePATH will help us achieve better coordination and information sharing among caregivers. Information will also be accessible wherever and whenever you need it, and you will have access to your patients’ CHP hospital/specialist records, including test results and chart notes. CarePATH also includes allergy and medication interaction warnings.
CarePATH will also result in enhanced convenience for you and your team through e-prescribing; in-basket communication; online display of test results to patients; preference lists for medications, procedures and chief complaints; and online scheduling and requests.
In addition, CarePATH will provide increased convenience for your patients through MyChart. MyChart enables enrolled patients to access their health information online, anytime, anywhere. MyChart is password protected and delivered via an encrypted connection, so all information is safe from unauthorized access. Online, patients can view medical information, stay in touch with their physician, manage appointments and access family health records.
